Abstract

아래턱뼈의 혀쪽면에서 관찰되는 덧구멍은 그 종류에 따라 빈도, 발현, 크기 및 분포가 다양하게 나타난다. 특히 이틀능선 가까이에 위치하여 수직의 방사선투과성 선으로 관찰되는 이틀사이구멍은 외과적 시술 시 다른 덧구멍에 비하여 손상으로 인한 합병증 발생의 위험이 더 크다. 따라서 본 연구에서는 콘빔전산화단층촬영 영상을 이용하여 미세 정상 해부학적 구조물인 이틀사이구멍에 대하여 조사하여 크기 및 성별과 연령에 따른 그 발현 양상을 분석하고자 하였다. 조선대학교 치과병원 영상치의학과에서 촬영한 총 110명의 콘빔전산화단층촬영 영상을 이용하였다. 이틀사이구멍의 발현되는 수, 빈도 및 치아를 기준으로 한 위치를 조사하였다. 이틀사이구멍의 직경, 아래턱뼈 아래모서리에서부터 이틀사이구멍의 위쪽 경계까지의 수직거리 및 이틀능선까지 아래턱뼈 높이를 계측하였다. 모든 계측 항목에서 성별과 연령에 따른 통계적 차이를 분석하였다. 이틀사이구멍은 110명의 콘빔전산화단층촬영 영상 중 98.1%에서 최소한 하나 이상이 관찰되었으며, 이들 대부분은 2개(41.8%) 또는 3개(40%)의 구멍을 가지고 있었다. 안쪽앞니와 가쪽앞니 사이에서 69.5%로 가장 높게 나타났으며, 다음으로 송곳니와 첫째작은어금니 사이에서 14.1%의 발현을 보였다. 이틀사이구멍의 평균 직경은 0.71±0.21 mm이고 평균 수직거리는 30.15±2.40 mm이며, 아래턱뼈의 평균 높이는 32.84 ±2.80 mm였다. 성별에 따라서는 유의적 차이가 나타나지 않았지만, 노년층에서 이틀사이구멍의 직경과 수직거리 및 턱뼈의 크기에 있어 유의적인 차이를 보였다. 1 mm 이상의 큰 직경을 가진 이틀사이구멍은 전체 대상자 중 26.4%에서 나타났으며, 특히, 노년층의 여성은 29.3%로 높은 발현을 보였다. 이러한 연구 결과는 상대적으로 안전한 지역인 아래턱 앞쪽부위에서 외과적 시술 시 쉽게 간과될 수 있는 미세 정상구조물인 이틀사이구멍에 관한 정량적 자료를 제시할 수 있다.

The interalveolar foramen, which is located close to the alveolar crest and observed as a vertical radiolucent line, has a greater risk of complications compared to other accessory foramen. Therefore, the aim of this study was to investigate the morphological characteristics of the interalveolar foramen and analyze them according to sex and age using cone beam computed tomography (CBCT) images. The number, frequency, and location of the interalveolar foramen were investigated on a total of 110 CBCT images. The diameter of the interalveolar foramen, the vertical distance to its upper margin, and the height to the alveolar crest of the mandible from the inferior border of the mandible were measured. There was at least one interalveolar foramen in 98.1% images, with 2 or 3 foramina in 81.8% images. It was present the most frequently between the central incisor and the lateral incisor in 69.5% images, and then between the canine and the first premolar in 14.1% images. The mean diameter of the interalveolar foramen was 0.71±0.21 mm and its mean vertical distance from the inferior border of the mandible was 30.15±2.40 mm. There was no significant difference according to sex, but there were significant differences in age in all measurement items. An interalveolar foramen with a diameter ≥ 1 mm appeared in 26.4% of all subjects, with elderly women showing a high incidence of 29.3%. These findings provide quantitative data on the interalveolar foramen that can be easily overlooked during surgical procedures in the anterior region of the mandible.
